Care Coordination

We offer a variety of services tailored to the needs of specific health populations in which we aim to coordinate and navigate individuals’ unique health care and health related social needs across the healthcare systems and home and community based services. We join hands alongside of those in need of services and support, helping people become more active in their own health and care, making the right connections, with the right teams at the right time.

These services empower, educate and advocate for individuals needing community resource assistance, health education and supports/services to allow them to remain as independent as possible in their homes and communities.

These services are referral-based programs and cannot be accessed from Senior Services directly. For more information about the referral-based programs please contact your health partners or healthcare insurance.
