We coordinate with health organizations to provide health and resource assessments to their members to identify needs, provide resources, connecting members with other health providers and community services to improve overall health outcomes.

How do I get connected to these services?

These services are not a service that an individual can request, they are referred through their health partners (ie: Veteran Affairs Medical Center (VAMC), specified insurance company)
