Chronic Disease Self-Management is a FREE evidence-based workshop for seniors with one or multiple chronic health conditions. If you or a loved one are living with a chronic condition such as arthritis, diabetes, heart disease, lung disease, or other ongoing health problems, this workshop is for you!

What to Expect:

I looked forward to my meetings

I wanted to thank you for being a great coach in this program. I personally feel that a program like this is very beneficial for everyone. Each week we met by phone, I found myself looking forward to our meetings. You taught skills that I can use in my everyday life. I have always set goals to achieve a better lifestyle financially but didn’t really make myself set personal goals for my health.

I took a look at my health

The meetings each week helped me to take a look at my “health” life. I wasn’t choosing the best road to follow. The Action Plans helped me become accountable to myself and the others in our small group. This guided me to do better each week. I also felt very comfortable, after a couple meetings, because you have health problems too. I felt the camaraderie. I didn’t feel I was dealing with chronic problems alone. I thank you for that.

My COPD was controlling my life

This program helped educate me by explaining how I can set small goals to start out and build on them to become healthier. My COPD was controlling my life. Our classes have taught me to make an action plan each week and build on it to become healthier. I’m taking short walks, light exercising each day, no snacks at night (chips), but maybe an apple or a veggie and quiet time before I go to bed. The book is very easy to read and understand. It has great guidelines for healthy eating, exercising, and nutritional facts.
